I can't help you on this but I've just found the same issue.
I have the extender within a formview that is itself on an update panel that is itself on a tabpanel
Would appreciate some guidance on this
Add a copy of the calendar css file in the toolkit, and add it to you App_Themes folder of your application, should fix this.

My calendar only shows the current month and current day, but not show the days of current month.
